Information about COVID-19 Contact Registers

As of Saturday 5 December 2020, each time you use a community hall or facility  ( excluding an oval or standalone toilet block) you will be required to ensure all persons 16 years and older, who are present in the community hall or facility during your booking time, register their attendance through a Contact Register.

Options for a Contact Register

Maintaining an attendance Contact Register can be done in one of two ways:

  1. By using the SafeWA app and scanning the QR code that has been placed in the facility by the Shire; or
  2. Using the hard copy contact register template below

Contact Register template

These must be returned to the Shire at the end of each week either by email (details on the register) or can be dropped into the Shire Administration Building. A copy will be recorded and only accessed if requested by the WA Health Department.

Further information about Contact Registers

  • All staff, volunteers and contractors must complete the register or scan the QR code.
  • This requirement to register is for all persons aged 16 years or over who enter the Shire halls/facilities, even if it is just for a short time such as to go to the toilet.
  • Persons under the age of 16 are not required to complete the contact register, although the FAQ’s to suggest that those under 16 years be encouraged to register ( or their parent to register them), to assist with contract tracing if required.
  • Should a person refuse to complete the register or register their attendance by scanning the QR code then you are able to refuse them entry to the facility.

We will be placing the QR codes at each of our halls and facilities prior to Saturday 5 December, should you notice the QR code is missing at any point please contact the Shire’s Facilities Team ASAP so that it can be replaced.
